We're using Subversion with a relatively inexpensive utility called "FogBugz." We have integrated it with Subversion and it is working well. I've not tried Bugzilla or any of the free options, but FogBugz is a very powerful issue tracker as well as reporting tool for trends and keeping track of items developers have on their plate. We also are going to start using it for requests for new work.
